Tier 1:  Individual Plan $129 a year 9am to 7pm Eastern Time support (no chronic) Includes text, email and as needed phone support excludes extended hours/holidays. Click Here to purchase

Tier 2:  Family Plan $249 a year 9am to 7pm Eastern Time support (no chronic) (for immediate family members in the home) Includes text, email and as needed phone support, excludes extended hours/holidays. Payment plans available. Click Here to purchase

Tier 3:  Premier Plan Individual:  $499 a year extended hours^ and Holidays* (see below) (no chronic) Includes database access, text, email and phone support. Includes wellness scans as needed pertaining to the current “acute issue”  as determined by Tonya. Payment plans available. Click Here to purchase

Tier 4:  Premier Plan Couple:  $599 a year extended hours^ and Holidays* (see below) (no chronic) Includes database access, text, email and phone support. ACUTE As Needed (No Plan)

​

Acute 1 time Support: $75 per incident per person (24-hour limit then, goes to normal hours support) includes text, email and as needed phone support during extended hours only (see below times, dates). Click Here to purchase

 

Acute 1 time Support AFTER HOURS:  $125 per incident per person (24-hour limit extended hours, then goes to normal hours support as listed below) includes 1 initial call and then, text, email and as needed phone support. Click Here to purchase

 

Definitions/Limitations:

​

^Extended Hours:  7am to 10pm Eastern Time and includes Tuesday and Sundays.

*Holidays: New Years Day, Memorial Day, July 4th, Labor Day, Thanksgiving, Christmas.  (Hours of service will be sent to active acute care clients)

 

Chronic/Acute:  Acute is 3/4 days or less and Chronic is over 3/4 days

 

Normal Business Hours:

 

Monday/Wednesday: 9am to 5pm

Tuesday/Sunday: Off

Thursday:  9am to 5pm weekly and 7pm-10pm by appointment (varies)

Friday:  9am to 2pm

Saturday:  by appointment only (varies)
